Degenerative intersegmental instability with compression of thecal sac and cauda equina.
Left image: The midline upright-flexion image shows a grade-one degenerative anterior spondylolisthesis at L4/5.
Right image: The midline upright-extension image reveals mobile spondylolisthesis at L4/5 with L4 translating posteriorly upon L5 in the sagittal plane. In addition, there is anterior infolding of the interspinous ligaments and ligamenta flava bilaterally resulting in severe stenosis of the central spinal canal at L3/4 and L4/5 and compression of the underlying thecal sac/cauda equina. Hyperintensity of the L4/5 interspinous space indicates ligamentous degenerative rupture of the interspinous ligament at this level resulting in the visualized degenerative hypermobile intersegmental instability.
